To ensure efficiency and to maximize your time in the visit it is reccomended that intake forms are submitted prior to your visit. Forms may be picked up from the office and dropped off at your convenience as well. Please ensure forms are filled out as honestly as possible as it helps form the complete picture of your current state of health. 

Please bring copies of all current and relevant lab work/imaging reports to your first visit if you have them available.

An initial patient visit is 75 minutes long, the focus is getting an understanding of your present state of health by asking many questions and doing a focused physical exam.  Initial treatment goals will be discussed.

 

A follow up visit is booked 3 - 4 weeks after your initial visit.  It is 30 minutes long.  In this visit we review the long term plan, discuss treatment options and follow up on the initial recommendations.

 

Visits after the initial follow up are case dependent and will be discussed in office.
